Ldh As Predictive And Recovery Parameter Of Dengue: Association With Ns1 And Igm Serological Markers

In the present study, association of LDH activity with serological markers of dengue infection was investigated by estimating serum NS1 antigen and IgM antibodies levels in dengue patients using capture ELISA followed by measurement of total LDH activity, and electropherotyping of LDH. NS1 and IgM parameters were compared with LDH levels, and analyzed statistically. The NS1 antigens were 35.23±5.37 U (normal <9U) with 634.67±350.55 U/L LDH (normal ≤250 U/L) (r=0.57) among 32.33±7.13 years patients and IgM antibodies were 52.38±7.68 U with 451.67±119.74 U/L LDH (r=0.13) among 25.92±5.01 years patients. The electropherotype showed more intense and greater number of band pattern when IgM was detected compared to NS1. The LDH profile showed LDH2>LDH3>LDH1>LDH4>LDH5 isoenzymes in descending order of intensity when IgM and NS1was detected. The NS1 titer increased proportionately with LDH; increased LDH activity was found when IgM was detected, because NS1 and IgM was detectable respectively from day one and five post-infection. Dengue is an acute infection with potential fatal complications. Specific antibody detection is an indirect method which is likely to produce both false positive and negative results. The novel parameter NS1 is reliable for diagnosis of dengue infection. Serum LDH is elevated in severe dengue on account of skeletal muscle damage and/or liver damage. The study attempts for investigation of total lactate dehydrogenase activity as an early prognostic and recovery marker of dengue.
